Health Assessment & Personal Exercise Program

Your comprehensive Health Assessment is your gateway to UQ Healthy Living’s services.

Designed to harness the benefits of interprofessional care, we take the time to understand your unique health journey from multiple perspectives, including the complexities of your circumstance and your goals.


Health Assessments

The initial Health Assessment is essential for your success and safety using the facility and informs your Health Action Plan and Personal Exercise Program, so that you get the most out of your experience with us.

What to wear
Comfortable clothing and exercise shoes will make it easier for you to complete the physical component of the assessment. You are welcome to use our change rooms, shower facilities and lockers before or after your assessment.

What to bring
Please also bring payment on the day. We accept cash, credit or debit card.

If you have been referred by a Team Care Arrangement you will need to bring your Medicare Card. Where applicable you will also require your Private Health or your Veterans Affairs card.

You may also like to bring a water bottle and a list of any medications you are currently taking.

What to expect
Allow one hour for your assessment. This will give you enough time to get to know some of our interprofessional team, and for them to get to know you.

You will be assessed by one to two student practitioners from any of our service disciplines. A qualified clinician may also be present. These students are trained to identify key contributors to your health and wellbeing and adhere to a thorough framework.

You will be asked a series of questions about your medical history, current medications, and lifestyle, including diet, exercise and social habits. Most importantly, we will listen to your concerns and intentions.

You are not compelled to answer any questions you are uncomfortable with; however, the accuracy of our recommendations is dependent on what you can disclose.

You will also be asked to perform a series of mild to moderate physical activities. You will be monitored during and after these exercises for your safety.

If at any time during this process you feel unwell, please notify our team immediately.

Health Action Plan
Using the information gained in your initial assessment, together we will establish a bespoke Health Action Plan.

This will consider any of our services that may be relevant to you, such as dietetics, clinical exercise physiology, physiotherapy,  occupational therapy, social work, counselling and psychology .

Your Health Action Plan may also prescribe additional exercise classes, and education seminars or workshops.

You’re in control
Your plan is obligation free. Access any of the services offered in your plan at your discretion, and avoid long term, lock-in contracts and commercial rates. Find our competitive fee schedule here.

Consider your plan just only the beginning of your journey with UQ Healthy Living. As your circumstances or interests change, and our service menu grows, you can request access to any of our services at any time.

Personal Exercise Program (PEP)

Your next appointment will take you through your Personal Exercise Program, incorporating a variety of exercises specifically designed to benefit your individual circumstances. This session is designed to ensure your safety and confidence following the program in Supported Exercise Sessions.

Many of the exercises prescribed will incorporate our user-friendly HUR Fitness Equipment. This world-leading technology is suitable for all fitness levels, with low starting loads of 100gm to ensure inclusivity. It employs pneumatic air resistance matched to the body’s natural muscular movement to reduce the risk of injury.

The UQ Healthy Living HUR SmartZone incorporates smartcard technology to automatically apply your individual pre-assessed resistance to the network linked exercise machines. It provides a full body workout in less than 30 minutes and collects data for your electronic record.

Express Reassessment

If you have been regularly participating in activities at the centre, we recommend a reassessment every six months to ensure you get the most out of our services.

The Express Reassessment will be used to update your exercise program to match your progress or any change in your condition. We may also offer additional services to you that apply.

If you have not engaged with the centre in three months or longer, you are required to undertake an Express Reassessment to ensure your program still applies.

Express Reassessments are typically not as in depth as your original assessment and generally take half an hour.
